“Severe arthritis in both hands, causing joint pain, diminished grip”

The key to this cipher is the File Number 153. To work out the code you must simply take a letter (or space) from the first line, then from the fifth line and then from the third line. Repeat this sequence until you have used all the letters and spaces to spell out the coded conditions and symptoms.
